Person Centred Planning
People with a learning disability often need extra support and services. An “assessment” is done to decide what support and services they should get.
This means taking a close look at their needs, their health and the way they live.
When doing assessments the thoughts and feelings of the person with a learning disability must always be listened to. So we have written a guide to “person centred assessments”.
Beth mae hwn yn ei drafod?
Yn aml mae ar bobl ag anabledd dysgu angen
cymorth a gwasanaethau ychwanegol. Gwneir
“asesiad” er mwyn penderfynu pa gefnogaeth a
pha wasanaethau y dylent eu cael. Mae hyn yn
golygu edrych yn fanwl ar eu hanghenion, eu
hiechyd a’u ffordd o fyw.
Wrth gynnal asesiad, rhaid gwrando ar feddyliau a
theimladau’r unigolyn gyda’r anabledd dysgu bob
tro. Felly rydym wedi llunio canllaw i “asesiadau
sy’n canolbwyntio ar y person”.
